BAYADA Home Health Care is seeking a Physical Therapist for a full-time home health opportunity based out of the Londonderry office, serving adult and geriatric clients in assigned territories. In this role, you function as part of a multi-disciplinary health care team delivering rehabilitative care and skilled nursing to help clients receive necessary medical treatment while remaining safely in their homes. A normal day in this position involves traveling to designated client locations to conduct home visits, performing physical evaluations and diagnostic tests, implementing customized care plans, teaching clients and families about rehabilitative practices, and recording observations using a tablet.
Responsibilities
- Conduct home visits to clients across assigned geographic territories.
- Conduct diagnostic tests and measurements including mobility, muscle strength, balance testing, and safety assessments.
- Apply protective interventions for wound prevention and healing using evidence-based dressings and agents.
- Create and execute personalized care plans featuring manual exercises, gait training, and balance interventions.
- Review and update care plans continually while collaborating with nursing staff on multidisciplinary care.
- Teach clients, family members, and representatives about rehabilitative care and safe independent living.
- Record observations, interventions, and evaluations accurately using a touch pad tablet.
Requirements
- A current New Hampshire Physical Therapist license.
- At least one year of recent work experience as a Physical Therapist.
- Graduation from an approved American Physical Therapy Association or Committee on Allied Health Education and Accreditation program.
- Capacity to work independently and manage time efficiently.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills.
