BAYADA Home Health Care is looking for dedicated Registered Nurses to join their team in northeast Massachusetts. In this full-time role, you will be part of a multi-disciplinary health care group that delivers skilled nursing and rehabilitative care, helping clients stay comfortably in their own homes with compassion and excellence. A typical day involves conducting head-to-toe patient assessments, managing acute and post-acute conditions, performing medication reconciliation, handling wound care, and independently navigating urgent clinical situations in home environments. You will also manage visit schedules, utilize point-of-care documentation tools, and collaborate closely with families and healthcare colleagues to provide reliable, high-quality care.
Responsibilities
- Perform head-to-toe patient assessments to detect subtle health changes
- Provide acute and post-acute care including wound care and IV therapy
- Triage and manage urgent clinical situations independently in homes
- Conduct medication reconciliation, teaching, and safety monitoring
- Perform comprehensive wound assessment, care, and documentation
- Manage a dynamic visit schedule and prioritize urgent visits
- Complete point-of-care documentation using technology efficiently
Requirements
- Current nursing license in good standing in Massachusetts
- Graduation from an accredited and approved nursing program
- At least one year of recent hands-on medical experience within the past two years
- Experience working with adult or geriatric populations in acute care, home health, or similar settings
- Ability to work independently and make real-time clinical decisions
- Strong problem-solving skills for unpredictable home environments
Good to have
- Experience in a Medicare Certified Home Health setting
- Familiarity with OASIS documentation
- Experience as a Start of Care or Admissions RN
